What is a Home Care Package?


A Home Care Package is a Commonwealth Government funded program that provides services to help you to remain at home for as long as possible. Packages range through four levels, from basic care to high care

Moyne Health Services is a recognised leader of Aged Care services with a 10 year history of providing Home Care Packages in the Moyne region. Our team of experienced and qualified staff aim to provide services to help you if you are having difficulties with your personal care or home care. We can provide you with options to assist you to remain at home for as long as possible with choices of care and services that are flexible as your needs change.

You can make choices about the type and delivery of your services, as well as which provider helps you and when.

How can you get a Home Care Package?


To be eligible to receive a Home Care Package you must be assessed and approved for a package by the Aged Care Assessment Services (ACAS). You can be referred to ACAS by contacting Moyne Health Services or through your GP, district nurse, hospital or health professional or you can refer yourself. Assessments are free of charge.

Once approved for a home care package, you can receive our services by contacting us directly, or you can be referred to us by your doctor, another health professional, a family member or a carer.
