What does a Credentialed Diabetes Educator do?

The basis of successful diabetes self-management is an understanding of the diabetes process and a sound knowledge of the treatment plan.

Credentialed Diabetes Educators work in partnership with General Practitioners, other members of the multidisciplinary team and the person with diabetes to achieve mutually agreed diabetes management goals.

The self-management model of care promotes access to and coordination of community health services to provide:

People with type 1 diabetes, type 2 diabetes (adult, paediatric) and gestational diabetes;
