Hospital Admission Risk Program (HARP)

HARP aims to prevent avoidable hospital presentations and admissions. The target group is people with complex care needs who would benefit from an holistic, integrated, person-centred approach to managing their conditions. 

HARP clinicians work in collaboration with other acute, community, aged care and specialist services.

HARP provides short term care coordination and self-management support to:

  • people with unstable chronic illnesses;
  • people with complex conditions due to ageing, or
  • people with psychosocial issues

Monday to Friday 08.00hrs – 17.00hrs
